For elective (non-trauma) surgeries, we make sure you have a thorough understanding of both surgical and non-surgical alternatives and help you choose options for treatment.
Appointments – What to Bring
Please bring your insurance cards, identification cards and any medical imaging records relating to your injury. Please contact your existing care provider as you will need to complete a medical records release with them.
For a shorter time in the waiting room, please fill out New Patient Packet and email it to us before your visit.
